Patients Consent To MedPerson Online Consultation.
I, HEREBY STATE THAT: By clicking on the link at the bottom of this page, that I have initiated a webcam video consultation with a doctor on MedPerson. I acknowledge that each consultation is via two-way video chat between a doctor and a patient (who in this case is myself).
I acknowledge that a consultation typically will last for 15 minutes and that I will not use abusive or offensive language during the consultation. I further acknowledge that I will forfeit the consultation fee that I have paid if I am verbally abusive to the doctor during the consultation.
I consent to MedPerson Limited and doctors on its online platform providing medical advice, prescriptions and other medical services that they deem necessary for my treatment and follow-up.
I accept that the doctor may send a push notification to my phone during the consultation and have made sure that my device is appropriately enabled.
I accept to be bound by the terms and conditions of service for using the MedPerson platform stated below:
SALES & REFUND TERMS AND CONDITIONS ("TERMS")
Services purchased are provided through the MedPerson platform at https://www.MedPerson.com/, or https://medperson.co.uk or https://medperson.netor the MedPerson app or on the phone with the MedPerson Contact Centre.
MedPerson services are provided by MedPerson Limited, which is a company incorporated in the United Kingdom with its registered office at 160 City Road, London EC1V 2NX, United Kingdom and with registered number 10620658 at the Companies House, Cardiff. To make it visually easier, the terms the MedPerson platform, the MedPerson app and the MedPerson Contact Centre refer to and is represented in this agreement as “MedPerson”, “we” or “us”. MedPerson may be contacted by mail at the registered address above, by telephone on 0843 886 8799 or by email at clinic@MedPerson.net.
You need to be at least 18 years old to place orders on the MedPerson platform.
Standard Cancellation Policy
You have the right to cancel your order or cancel a service contract for any reason. To cancel a service contract, please send your intent to return or cancel to us firstname.lastname@example.org before you have used the service as this is a digital product. No cancellations after you have used the service will be accepted and registration fees are non-refundable.
If at anytime prior to14 days after the date you, or a person indicated by you, purchased the service(s), or from the date of conclusion of the service contract. If you ordered multiple services in the same order, please send your intent to cancel to us anytime prior to the date 14 days after the date you, or a person indicated by you, ordered the service. To exercise these rights, you need only provide us with a clear expression of your intent to cancel a service contract or cancel an order. You can notify us by either sending the model form found at the end of this Policy, submitting the electronic form found online at
https://www.MedPerson.com/contact, calling our telephone number on 0843 866 8799 or sending an email to accounts@MedPerson.net.
Once you have notified us of your intention to cancel, we will provide you with a refund based on the original payment method within 14 days. Please note that you may not return the following, except in case of defect or lack of conformity:
- Electronic software downloads when download has begun with your prior express consent and acknowledgement that you lose your right of withdrawal.
- Services, like medical consultations and specialist opinion, after the services have been fully performed if performance has begun with you prior express consent and acknowledgement that you lose your right of withdrawal once the services have been fully performed.
If services have not been performed, and you have requested performance to begin within 14 days from the date of conclusion of the service contract, you will be charged an amount proportional to the services rendered until the time you have informed us of your intent to cancel the service contract.

Some MedPerson services utilise wireless services that may incur extra costs and are governed by the terms of a separate agreement between you and your selected wireless service provider.
Pricing and Price Reductions/Corrections
MedPerson reserves the right to change prices for services offered on the MedPerson platform at any time and to correct pricing errors that may inadvertently occur. Any price changes will not affect an order that has already been accepted by MedPerson. In the event of a pricing error, we will notify you of the error and allow you to either proceed with the transaction at the correct price or cancel your order at no cost to you. Additional information about pricing and taxes is available in the Payment & Pricing section of our website located at https://www.MedPerson.net/payments.
In addition to your right to cancel your order or cancel a service contract for any reason as explained above, should MedPerson reduce its price on any MedPerson-branded product within 14 calendar days from the date you receive your product, feel free to contact the MedPerson Store by telephone on 0843 886 8799 to request a refund or credit of the difference between the price you were charged and the current selling price. To receive the refund or credit, you must contact MedPerson within 14 calendar days of the price change. Please note that this excludes limited-time price reductions, such as those that occur during special sales events.
Order, Order Acknowledgement and Order Acceptance
To place an order for services on MedPerson, you can visit the website at https://www.MedPerson.com/ or use the MedPerson app, and then simply add the services you wish to your shopping cart, check out (using your chosen payment method) with your MedPerson ID or as a guest customer, and have your services will start. Alternatively, you can call the MedPerson telephone number on 0843 886 8799 and a MedPerson Customer Services Officer will assist you through the entire order process, from service selection to payment method. Whichever way you choose to order, you will be given an opportunity to review your order prior to submitting it. Once you have placed your order, we will provide you with a web order number.
Once we receive your order, we’ll email you an order confirmation. As with the web order number, the order confirmation email does not signify our acceptance of your order; we are simply confirming that we received your order. Once we have processed your order and your service will start, we will send you an email stating that we have accepted your order, which will include the service start details (the “Servicet Notification Email”). The decision to accept your order is in our sole discretion. Because we want to make sure that you are in a jurisdiction in which the service that you have chosen is available, we won’t accept your order until we have confirmed this. In most cases, this will be within ten minutes for services that are available. For items that are unavailable, you will receive the Order Acceptance Email when the service becomes available, which in no event will be later than the “48 hours”. Once we have sent and you have received the Order Acceptance Email, MedPerson will have formed a contract with you (we will include the key terms of the contract in this acceptance email). All that remains at that point is for you to start using the service(s) provision for you, which we will do straightaway
The MedPerson platform offers services via an online tool. The delivery is online for all orders. Visit https://www.MedPerson.com/to learn more about how and when you will receive the services you purchased from MedPerson. Please note that the actual delivery of your order can be impacted by many events beyond MedPerson’s control including the availability of doctors in the jurisdiction that you are located. If there are no doctors available in your jurisdiction, we may give you the option of consulting with another doctor in a nearby jurisdiction that is capable of providing the consultation. We will always notify you of any significant delays, and work with you to ensure a smooth service.

This contract is subject to applicable export control laws. You may not export any service purchased from the MedPerson to another country if prohibited by export control laws. If you do plan to export an MedPerson product to another country, you must obtain any legally required export licences (or other government approvals) prior to doing so.
Service Availability and Limitations
Given the popularity and/or supply constraints of some of our services, MedPerson may have to limit the number of services available for purchase. MedPerson reserves the right to change quantities available for purchase at any time, even after you place an order, provided MedPerson has not yet accepted your order.
MedPerson offers the following payment methods: credit card and bank transfer. If you are paying by credit card, when you submit your order, we may block up to the total amount of the order on your credit card. If we do not receive authorisation from your credit card issuer, we will inform you so that you can provide us with another payment method. Your credit card will be charged once we accept your order. We reserve the right to verify the identity of the credit card holder by requesting appropriate documentation. To ensure that your credit, debit or charge card is not being used without your consent, we will validate name, address and other personal information supplied by you during the order process against appropriate third-party databases. By accepting these terms and conditions, you consent to such checks being made. In performing these checks, personal information provided by you may be disclosed to registered Credit Reference Agency, which may keep a record of that information. You can rest assured that this is done only to confirm your identity, only in connection with purchases made by credit, debit or charge card (and excluding the case where you have requested finance in order to pay for the services ordered), that a credit check is not performed and that your credit rating will be unaffected. All information provided by you will be treated securely and strictly in accordance with the UK Data Protection Act 1998 and European GDPR 2018.
If you are paying by bank transfer, we will send you a Proceed to Pay email after you have placed your order. Once we have received your payment, we will send you an order confirmation email advising that payment has been received and your order is being processed. Once we have processed your order and prepared it for shipping, we will send you an email stating that we have accepted your order and including details of shipment. If we do not receive payment within 30 days after your receipt of the Proceed to Pay email, we will cancel your order.

You agree not to exploit the Site, or any Services, MedPerson Events or Content provided to you by MedPerson as a MedPerson patient, in any unauthorised way, including but not limited to, by trespass, burdening network capacity or using the Services, Site or Content other than for authorised purposes. Copyright and other intellectual property laws protect the Site and Content provided to you, and you agree to abide by and maintain all notices, license information, and restrictions contained therein. Unless expressly permitted herein or otherwise permitted in a separate agreement with MedPerson, you may not modify, publish, network, rent, lease, loan, transmit, sell, participate in the transfer or sale of, reproduce, create derivative works based on, redistribute, perform, display, or in any way exploit any of the Site, Content or Services. You may not decompile, reverse engineer, disassemble, or attempt to derive the source code of any software or security components of the Services, Site, or Content (except as and only to the extent any foregoing restriction is prohibited by applicable law or to the extent as may be permitted by any licensing terms accompanying the foregoing). Use of the Site, Content or Services to violate, tamper with, or circumvent the security of any computer network, software, passwords, encryption codes, technological protection measures, or to otherwise engage in any kind of illegal activity, or to enable others to do so, is expressly prohibited. MedPerson retains ownership of all its rights in the Site, Content, MedPerson Events and Services, and except as expressly set forth herein, no other rights or licenses are granted or to be implied under any MedPerson intellectual property.
Except as otherwise set forth herein, you agree that any MedPerson electronic documents, patient records, ePrescriptions, laboratory tests and results, radiology requests and results, medical reports, software, services, and/or hardware (including related documentation and materials) provided to you as an MedPerson registered patient and any information disclosed by MedPerson to you in connection with the MedPerson Online Clinic will be considered and referred to as “MedPerson Confidential Information ”.
Notwithstanding the foregoing, MedPerson Confidential Information will not include: (a) information that is generally and legitimately available to the public through no fault or breach of yours; (b) information that is generally made available to the public by MedPerson; (c) information that is independently developed by you without the use of any MedPerson Confidential Information; (d) information that was rightfully obtained from a third party who had the right to transfer or disclose it to you without limitation; or (e) any third party documentation provided to you by MedPerson and accompanied by licensing terms that do not impose confidentiality obligations on the use or disclosure of such documentation.
Nondisclosure and Non-use of MedPerson Confidential Information .
Unless otherwise expressly agreed or permitted in writing by MedPerson, you agree not to disclose, publish, or disseminate any MedPerson Confidential Information to anyone other than to Doctors practicing on the MedPerson platform or your family members and carers and then only to the extent that MedPerson does not otherwise prohibit such disclosure. Except for your authorised purposes as an MedPerson registered patient or as otherwise expressly agreed or permitted by MedPerson in writing, you agree not to use MedPerson Confidential Information in any way, including, without limitation, for your own or any third party’s benefit without the prior written approval of MedPerson in each instance. You further agree to take reasonable precautions to prevent any unauthorised use, disclosure, publication, or dissemination of MedPerson Confidential Information. You acknowledge that unauthorised disclosure or use of MedPerson Confidential Information could cause irreparable harm and significant injury to MedPerson that may be difficult to ascertain. Accordingly, you agree that MedPerson will have the right to seek immediate injunctive relief to enforce your obligations under this Agreement in addition to any other rights and remedies it may have.
If you are required by law, regulation or pursuant to the valid binding order of a court of competent jurisdiction to disclose MedPerson Confidential Information, you may make such disclosure, but only if you have notified MedPerson before making such disclosure and have used commercially reasonable efforts to limit the disclosure and to seek confidential, protective treatment of such information. A disclosure pursuant to the previous sentence will not relieve you of your obligations to hold such information as MedPerson Confidential Information.
Confidential Materials License and Restrictions.
If MedPerson provides you with Pre-Release Materials, then subject to your compliance with the terms and conditions of this
Agreement, MedPerson hereby grants you a nonexclusive, nontransferable, right and license to use the Pre-Release Materials only for the limited purposes set forth in this Section; provided however that if such Pre-Release Materials are subject to a separate license agreement, you agree that the license agreement accompanying such materials in addition to the Sections on Nondisclosure Nonuse of MedPerson Confidential Information of this Agreement shall also govern your use of the Pre-Release Materials. You further agree that in the event of any inconsistency between this Section of this Agreement and the confidentiality restrictions in the license agreement, the license agreement shall govern. This Agreement does not grant you any right or license to incorporate or make use of any MedPerson intellectual property (including for example and without limitation, trade secrets, patents, copyrights, trademarks and industrial designs) in any product. Except as expressly set forth herein, no other rights or licenses are granted or to be implied under any MedPerson intellectual property. You agree not to decompile, reverse engineer, disassemble, or otherwise reduce the Pre-Release Materials to a human-perceivable form, and you will not modify, network, rent, lease, transmit, sell, or loan the Pre-Release Materials in whole or in part.
Registered Patient’s Content Restrictions.
As a MedPerson registered patient, you may have access to certain proprietary content (including, without limitation, video presentations and audio recordings) that MedPerson may make available to you from time to time (“ Content”). Content shall be considered MedPerson Confidential Information, unless otherwise agreed or permitted in writing by MedPerson. You may not share the Content with anyone, including, without limitation, family and friends or carers, regardless of whether they are MedPerson patients, unless otherwise expressly permitted by MedPerson. Subject to these terms and conditions, MedPerson grants you a personal and nontransferable license to access and use the Content for authorized purposes as a MedPerson registered patient; provided that you may only download one (1) copy of the
Content and such download must be completed within the time period specified by MedPerson for such download. Except as expressly permitted by MedPerson, you shall not modify, translate, reproduce, distribute, or create derivative works of the Content or any part thereof. You shall not rent, lease, loan, sell, sublicense, assign or otherwise transfer any rights in the Content. MedPerson and/or MedPerson’s licensor(s) retain ownership of the Content itself and any copies or portions thereof. The Content is licensed, not sold, to you by MedPerson for use only under this Agreement, and MedPerson reserves all rights not expressly granted to you. Your rights under this license to use and access the Content will terminate automatically without notice from MedPerson if you fail to comply with any of these provisions.
You agree that any Materials that MedPerson provides as part of its Patients Education Services are licensed to you and shall be used by you only in accordance with the terms and conditions accompanying the Materials. MedPerson retains ownership of all of its right, title and interest in such Materials and no other rights or licenses are granted or to be implied under any MedPerson intellectual property.
You have no right to copy, decompile, reverse engineer, sublicense or otherwise distribute such Materials, except as may be expressly provided in the terms and conditions accompanying the Materials. YOU AGREE THAT WHEN REQUESTING AND RECEIVING TECHNICAL SUPPORT FROM PATIENTS EDUCATION SERVICES, YOU WILL NOT PROVIDE MEDPERSON WITH ANY INFORMATION, INCLUDING THAT INCORPORATED IN YOUR SOCIAL HISTORY, THAT IS CONFIDENTIAL TO YOU OR ANY THIRD PARTY. YOU AGREE THAT ANY NOTICE, LEGEND, OR LABEL TO THE CONTRARY CONTAINED IN ANY SUCH MATERIALS PROVIDED BY YOU TO MEDPERSON SHALL BE WITHOUT EFFECT. MEDPERSON SHALL BE FREE TO USE ALL SUCH INFORMATION IT RECEIVES FROM YOU IN ANY MANNER IT DEEMS
APPROPRIATE FOR YOUR TREATMENT, SUBJECT TO ANY APPLICABLE PATENTS OR COPYRIGHTS.
MedPerson reserves the right to reject a request for access to Labs or for Patients Education Services at any time and for any
reason, in which event MedPerson may credit you for the rejected lab or support request. You shall be solely responsible for any restoration of lost or altered files, data, programs or other materials provided.
MedPerson reserves the right, at its discretion, to modify this Agreement, including any rules and policies at any time. You will be responsible for reviewing and becoming familiar with any such modifications (including new terms, updates, revisions, supplements, modifications, and additional rules, policies, terms and conditions)(“ Additional Terms”) communicated to you by MedPerson. All Additional Terms are hereby incorporated into this
Agreement by this reference and your continued use of the Site will indicate your acceptance of any Additional Terms. In addition, MedPerson may be sending communications to you from time to time. Such communications may be in the form of phone calls and/or emails and may include, but not be
limited to, membership information, clinical information relevant to your medical history, technical information, and updates and/or
changes regarding your participation as an MedPerson Registered Patient. By agreeing to this Agreement, you consent that MedPerson may provide you with such communications.
Term and Termination.
MedPerson may terminate or suspend you as a MedPerson Registered Patient at any time at MedPerson’s sole discretion. If MedPerson terminates you as a MedPerson Registered Patient, MedPerson reserves the right to deny your reapplication at any time in MedPerson’s sole discretion. You may terminate your participation as a MedPerson Registered Patient at any time, for
any reason, by notifying MedPerson in writing of your intent to do so. Upon any termination or, at MedPerson’s discretion, suspension, all rights and licenses granted to you by MedPerson will cease, including your right to access the Site, and you agree to destroy any and all MedPerson Confidential Information that is in your possession or control. At MedPerson’s request, you agree to provide certification of such destruction to MedPerson. No refund or partial refund of any fees paid hereunder or any other fees will
be made for any reason.
MedPerson Independent Patients.
Nothing in this Agreement will impair MedPerson’s right to cultivate, acquire, license, market, promote or distribute services, to your friends, family carers or others involved with you previously. In the absence of a separate written agreement to the contrary, MedPerson will be free to use any information, suggestions or recommendations you provide to MedPerson pursuant to this Agreement for any purpose, subject to any applicable patents or copyrights.
Use Of MedPerson Trademarks, Logos, etc.
You agree to not to use any MedPerson’s trademarks, logos or intellectual property except as expressly authorised in writing by MedPerson in each instance or as permitted in this agreement.
You agree that all goodwill arising out of your authorised use of MedPerson’s marks shall inure to the benefit of and belong to MedPerson.
MEDPERSON AND ITS AFFILIATES, SUBSIDIARIES, OFFICERS, DIRECTORS, EMPLOYEES, AGENTS, PARTNERS, AND LICENSORS (COLLECTIVELY, “ MEDPERSON” FOR
PURPOSES OF THIS SECTION) DO NOT PROMISE THAT THE SITE, CONTENT, SERVICES (INCLUDING, FUNCTIONALITY OR FEATURES OF THE FOREGOING), LABS, PATIENTS EDUCATION SERVICES, OR ANY OTHER INFORMATION OR MATERIALS THAT YOU RECEIVE HEREUNDER AS AN MEDPERSON REGISTERED PATIENT (COLLECTIVELY, THE “ SERVICE” FOR PURPOSES OF THIS SECTION) WILL BE ACCURATE, RELIABLE, TIMELY, SECURE, ERROR-FREE OR UNINTERRUPTED, OR THAT ANY DEFECTS WILL BE CORRECTED. THE SERVICE IS PROVIDED ON AN “AS-IS” AND “AS-AVAILABLE” BASIS AND THE SERVICE IS SUBJECT TO CHANGE WITHOUT NOTICE. MEDPERSON CANNOT ENSURE THAT ANY CONTENT (INCLUDING FILES, INFORMATION OR OTHER DATA) YOU ACCESS OR DOWNLOAD FROM THE SERVICE WILL BE FREE OF VIRUSES, CONTAMINATION OR DESTRUCTIVE FEATURES. FURTHER, MEDPERSON DOES NOT GUARANTEE ANY RESULTS OR IDENTIFICATION OR CORRECTION OF PROBLEMS AS PART OF THE SERVICE AND MEDPERSON
DISCLAIMS ANY LIABILITY RELATED THERETO. MEDPERSON DISCLAIMS ALL WARRANTIES, EXPRESS OR IMPLIED, INCLUDING ANY WARRANTIES OF ACCURACY, NONINFRINGEMENT, MERCHANTABILITY AND FITNESS FOR A PARTICULAR PURPOSE. MEDPERSON
DISCLAIMS ANY AND ALL LIABILITY FOR THE ACTS, OMISSIONS AND CONDUCT OF ANY THIRD PARTIES IN CONNECTION WITH OR RELATED TO YOUR USE OF THE SERVICE.
YOU ASSUME TOTAL RESPONSIBILITY AND ALL RISKS FOR YOUR USE OF THE SERVICE, INCLUDING, BUT NOT LIMITED TO, ANY INFORMATION OBTAINED THEREON. YOUR SOLE
REMEDY AGAINST MEDPERSON FOR DISSATISFACTION WITH THE SERVICE IS TO STOP USING THE SERVICE. THIS LIMITATION OF RELIEF IS A PART OF THE BARGAIN BETWEEN THE PARTIES. TO THE EXTENT THAT MEDPERSON MAKES ANY PRODUCTS, SERVICES OR INFORMATION RELATED THERETO AVAILABLE TO YOU AS AN MEDPERSON REGISTERED PATIENT, YOU UNDERSTAND THAT MEDPERSON IS UNDER NO OBLIGATION TO PROVIDE CLINICAL UPDATES, ENHANCEMENTS, OR CLINICAL OPINIONS, OR TO NOTIFY YOU OF ANY PRODUCT OR SERVICES CHANGES THAT MEDPERSON MAY MAKE, OR
TO PUBLICLY ANNOUNCE OR INTRODUCE THE PRODUCT(S) OR SERVICE AT ANY TIME IN THE FUTURE.
Disclaimer of Liability.
TO THE EXTENT NOT PROHIBITED BY APPLICABLE LAW, IN
NO EVENT WILL MEDPERSON BE LIABLE FOR PERSONAL INJURY, OR ANY INCIDENTAL, SPECIAL, INDIRECT, CONSEQUENTIAL OR PUNITIVE DAMAGES WHATSOEVER,
INCLUDING, WITHOUT LIMITATION, DAMAGES RESULTING FROM DELAY OF DELIVERY, FOR LOSS OF PROFITS, DATA, HEALTH BENEFITS OR GOODWILL, FOR BUSINESS INTERRUPTION OR ANY OTHER COMMERCIAL DAMAGES OR LOSSES, ARISING OUT OF OR RELATED TO THIS AGREEMENT OR YOUR USE OR INABILITY TO USE THE SERVICE, HOWEVER CAUSED, WHETHER UNDER A THEORY OF CONTRACT, WARRANTY, TORT (INCLUDING NEGLIGENCE), PRODUCTS LIABILITY, OR OTHERWISE, EVEN IF MEDPERSON HAS BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES, AND NOTWITHSTANDING THE FAILURE OF ESSENTIAL PURPOSE OF ANY REMEDY. IN NO EVENT SHALL MEDPERSON’S TOTAL LIABILITY TO YOU UNDER THIS AGREEMENT FOR ALL DAMAGES (OTHER THAN AS
MAY BE REQUIRED BY APPLICABLE LAW IN CASES INVOLVING PERSONAL INJURY)
EXCEED THE AMOUNT OF FIFTY POUNDS (£50.00).
Third-Party Notices and Products.
Third-party software provided by MedPerson to you as part of
MEDICAL EDUCATION SERVICES may be accompanied by its own licensing terms, in which case such licensing terms will govern your use of that particular third-party software. Mention of third-parties and third-party products in any materials, documentation, advertising, or promotions provided to you as an
MedPerson Registered Patient is for informational purposes only and constitutes neither an endorsement nor a recommendation. All third-party product specifications and descriptions are supplied by the respective vendor or supplier, and MedPerson shall have no responsibility with regard to the selection, performance, or use of these vendors or products. All understandings, agreements, or warranties, if any, take place directly between the vendors and the prospective users.
You may not use or otherwise export or re-export any MedPerson Confidential Information received from MedPerson except as authorised by the laws of England & Wales and the laws of the jurisdiction in which the MedPerson Confidential Information was obtained. In particular, but without limitation, the MedPerson Confidential Information may not be exported or re-exported (a) into any UK embargoed countries or (b) to anyone on the U.S. Treasury Department's list of Specially Designated Nationals or the U.S. Department of Commerce Denied Person's List or Entity List or any other restricted party lists. By becoming an MedPerson Registered Patient or using any MedPerson Confidential Information, you represent and warrant that you are not located in any such country or on any such list. You also agree that you will not use any MedPerson Confidential Information for any purposes prohibited by United Kingdom law, including, without limitation, the development, design, manufacture or production of nuclear, chemical or biological weapons.
This Agreement will be governed by and construed in accordance with the laws of England and Wales, excluding its conflict of law provisions. The parties further submit to and waive any objections to personal jurisdiction of and venue in any of the following forums:
Central London High Court, United Kingdom, or any other High Court in London, for any disputes arising out of this Agreement.
No delay or failure to take action under this Agreement will constitute a waiver unless expressly waived in writing, signed by a duly authorised representative of MedPerson, and no single waiver will constitute a continuing or subsequent waiver. This Agreement will bind your successors but may not be assigned, in whole or part, by you without the written approval of an authorised representative of MedPerson. Any non-conforming assignment shall be null and void. If any provision is found to be unenforceable or invalid, that provision shall be limited or eliminated to the minimum extent necessary so that this Agreement shall otherwise remain in full force and effect and enforceable. This Agreement constitutes the entire agreement between the parties with respect to its subject matter and supersedes all prior or contemporaneous understandings regarding such subject matter. No addition to or removal or modification of any of the provisions of this Agreement will be binding upon MedPerson unless made in writing and signed by an authorised representative of MedPerson. The parties hereto confirm that they have requested that this Agreement and all attachments and related documents be drafted in English. I understand that information about my MedPerson consultation will be recorded and processed on computer in order to progress and monitor appointments. I also understand that my medical records will be stored online. I consent to the recording, processing and storing of personal data in this way in accordance with Data Protection Act 1998 as amended in 2018. I accept that my data may be transferred across jurisdictions and stored in different countries when necessary for the effective management of my clinical condition.
**I authorise MedPerson Limited and their doctors to release any information obtained during my consultation and management to my healthcare insurer or other payer.
